Tranexamic acid

Also listed as TXA and Cetyl tranexamate mesylate (derivative)

Moderate evidence 2–5% topical

Tranexamic acid is a synthetic amino-acid derivative, used orally in medicine to reduce bleeding, that has become a popular topical ingredient for stubborn discoloration. Topically it is studied mainly for melasma-type pigmentation and dark spots.

What it does

The short version.

Tranexamic acid interferes with plasmin activity in the skin, which in turn dampens signals that stimulate melanocytes to make pigment; it may also reduce the small blood vessels associated with melasma. Topical serums and creams aim to fade brown patches and post-blemish marks gradually, typically over two to three months, and it is often combined with niacinamide or other brighteners.

Moderate evidence

Small randomized trials and an expert consensus support topical TXA for melasma, often with fewer side effects than hydroquinone; studies use varied concentrations and vehicles and are short. Oral TXA has stronger data but is prescription-only.

How to use it

Getting it right.

Apply once or twice daily to the whole area of concern after cleansing and before moisturizer, with strict daily sunscreen, since UV re-darkens pigmentation.

Before you start

Topical use is generally well tolerated, with occasional mild irritation or dryness. Melasma is a medical condition that can return; see a dermatologist for diagnosis and for prescription options, including oral TXA, which carries clotting-related cautions.
